ID Cards


Medical

If you enroll in a different medical plan for 2021, you will receive a new ID card(s) from Blue Cross Blue Shield of Illinois, otherwise, you will continue using your current card. If you need additional ID cards, contact Blue Cross Blue Shield at 1-877-224-9598 or visit http://www.bcbsil.com/foundations.

 

As a reminder, this card is used for both medical and prescription coverage/services.


Dental

If you enroll in a different dental plan for 2021, you will receive a new Delta Dental of Kansas ID card(s), otherwise, you will continue using your current card. If you need additional ID cards, contact Delta Dental at 1-800-234-3375 or visit www.deltadentalks.com.


PayFlex

Debit cards remain active for 5 years. If the card expires, PayFlex will issue a new card. If you dispose your old card, you will need to contact PayFlex to request a new card be issued.

Switching from FSA to HSA

  • If you previously had the debit card option, you will use the same debit card. However, effective January 1, 2021, the card will default to use HSA contributions only.
  • The debit card cannot be used after December 31, 2020, for FSA claims. You have until March 31, 2021, to submit a claim online or via fax for claims incurred in 2020. 

Switching from HSA to FSA

  • If you elect the FSA, you will use the same debit card. However, effective January 1, 2021, the card will default to use FSA contributions only.
  • If you want to use your HSA funds, you will need to submit a claim online.
  • When no longer contributing, the HSA moves to an individual "retail" account. You should receive a notification of this change in your account and that you will be charged a monthly maintenance fee.


Electing a Limited Purpose FSA

  • If you elect the LPFSA, you will use your PayFlex HSA card which will manage both your HSA/LPFSA accounts.
  • Using your card for a dental or vision expense will pull your LPFSA funds first; all other eligible expenses will be pulled from the HSA.

If you need an additional debit card, you can contact PayFlex at 1-800-284-4885 or by logging in to your PayFlex account and choose 'Request Dependent Card' option. Mailing time for the cards is 10-15 business days.


 

Vision Service Plan

No ID card is associated with this plan. In-network providers are able to confirm coverage with VSP directly.
