The Premium PPO plan offers flexibility to see any health care provider you choose. However, when you go to an in-network BlueCross BlueShield provider, you are going to receive services at a negotiated discounted fee.
Locate a Network Provider
BCBSIL has a large network of providers. You can find a list of participating providers by logging on to or enrolling in your account at www.bcbsil.com/foundations, or by calling BCBSIL at 1-866-563-8366. Also, call you doctor and ask, “Do you accept BlueCross BlueShield?”
How do I Make Changes to My Plan?
Changes to your pre-tax benefit elections – outside of your original eligibility period or annual enrollment – are only allowed in the event of a qualifying change in status. Changes to your benefit elections due to a life-changing event must be made within 31 days of the event. Learn More ...
