Birth/Adoption
Life events such as a birth, adoption or gaining a dependent allow you to make changes to your benefits plan. Depending on the life event, you may need to add or remove a dependent from your benefit plans. You have 60 days from the birth or adoption of dependent to make changes to your benefit plans.
How to Make Changes to Your Benefits Plan
- Complete the Status Change Online Form.
- Once your benefit elections have been submitted and reviewed, you will receive an e-mail notification.
- Be sure to review your benefit summary, which is available on Ceridian Self-Service.
- Review your pay advice to confirm your benefit deductions.
How to Add a Dependent to Your Benefits Plan

Effective Date
The date of the event in which the child date of birth or the official adoption date.

Vendor Notification
Completed electronically once the health care vendor is notified of the dependent's coverage; any claims submitted will automatically be processed.

Personal Information
You may need to update or change some personal information by logging on to Self-Service.
- Marital status (updating marital status does not add or remove your spouse from your health and welfare plan; you must complete a status change form to add or remove a dependent(s) from your benefit plans)
- Dependent information (adding or removing a dependent in Self-Service does not add or remove your dependent from your health and welfare plan; you must complete a status change form to add or remove dependent(s) from your benefit plans)
- Address/phone change
- Name change (requires updated Social Security card)
- Update emergency contact information
Already have Family or EE + Children Coverage?
You still need to complete and submit a status change form to add your dependent to your health care plans.
Need Additional Life Insurance?
You have 31 days from the birth or adoption date of dependent to elect Dependent Term Life. You can also elect GUL and Optional Life at the guaranteed issue amount.
