Domestic Partner

Farm Credit Foundations offers medical, dental and vision coverage for domestic partners.  Domestic partners are defined as two unmarried adults at least 18 years of age of the same or opposite sex that are not related by blood that have lived together for more than six months in a exclusive committed relationship of mutual caring and support. The domestic partner’s eligible children may be enrolled as long as the domestic partner is enrolled and the children’s legal residence is the home of the employee.

 

How to Make Changes to Your Benefits Plan

  1. Complete the Status Change Online Form.
  2. Once your benefit elections have been submitted and reviewed, you will receive an e-mail notification.
  3. Complete the required Affidavit*.
  4. Be sure to review your benefit summary, which is available on Ceridian Self-Service.
  5. Review your pay advice to confirm your benefit deductions.

*Affidavit Requirements

  • Must be signed by employee and domestic partner and notarized.
  • You will be notified if your application meets criteria or additional information required.
  • If requested, two forms of the following types of documentation may be required:
    • Joint title, mortgage or lease
    • Joint ownership of motor vehicle, joint checking account or joint credit account
    • Designation of domestic partner as primary beneficiary in will, life insurance and/or retirement benefits
    • Proof of domestic partner power of attorney
    • Other documentation that establishes formal relationship

Eligibility of a Domestic Partner

The employee and domestic partner must meet the following criteria:

  • Have lived together for at least six months and intend to do so indefinitely.
  • Are not married to anyone else nor have another domestic partner.
  • Are at least 18 years of age and mentally competent to consent to contract.
  • Employee and domestic partner cannot be related by blood closer than would bar marriage in the state of residence.
  • Jointly responsible for each other's common welfare and share financial obligations.
