Flu Season is Not Over! H1N1 Flu Shots Available

BlueCross BlueShield of Illinois will cover flu shots, including the H1N1 vaccine, administered through any provider type. This includes shots received at a drug store, grocery store, public health office, etc.
If you received a shot outside of your clinic, you may file a claim for reimbursement under our BCBSIL PPO plans.
You will need to complete the claim form and attach a receipt containing:
- Date of service
- Type of service (flu vaccination)
- Provider information (i.e. pharmacy name, phone number and tax ID if available)
- Name of person receiving the shot (patient)
- Cost of the shot: BlueCross BlueShield will reimburse you up to $15 for the cost of the shot
