Domestic Partner


Farm Credit Foundations offers medical, dental and vision coverage for domestic partners. Domestic partners are defined as two unmarried adults at least 18 years of age of the same or opposite sex that are not related by blood that have lived together for more than six months in a exclusive committed relationship of mutual caring and support.


The domestic partner’s eligible children may be enrolled as long as the domestic partner is enrolled and the children’s legal residence is the home of the employee.

A qualifying status change allows you to add or remove a dependent from your existing benefit coverage (as applicable). Any changes made to coverage must be consistent with the Qualified Status Change under IRS Section 125.

IMPORTANT: Changes to your benefit elections due to a life-changing event must be made within 31 days of the event.


 

Eligibility of a Domestic Partner

The employee and domestic partner must meet the following criteria:

  • Have lived together for at least six months and intend to do so indefinitely.

  • Are not married to anyone else nor have another domestic partner.

  • Are at least 18 years of age and mentally competent to consent to contract.

  • Employee and domestic partner cannot be related by blood closer than would bar marriage in the state of residence.

  • Jointly responsible for each other's common welfare and share financial obligations.

How to Make Changes to Your Benefits Plan

  1. Complete the Life Event Declaration in Dayforce. After you log in to Dayforce, look for the form under Benefits > Forms

  2. Once the form has been submitted, an election window will open for you to make your benefit changes. This will be viewable under Benefits > Overview (click refresh if it is not displaying).

  3. Your elections will not be processed until you click "Submit Enrollment".

  4. After you submit your elections, review and print your confirmation statement.

  5. Farm Credit Foundations will review all submitted changes within three business days. If an error is identified, you will be notified in the message center within Dayforce.

  6. Complete the required Affidavit* and send to benefits@farmcreditfoundations.com.

  7. Review your pay advice to confirm your benefit deductions are reflecting accurately.

*Affidavit Requirements

  • Must be signed by employee and domestic partner and notarized.

  • You will be notified if your application meets criteria or additional information required.

  • If requested, two forms of the following types of documentation may be required:

    • Joint title, mortgage or lease

    • Joint ownership of motor vehicle, joint checking account or joint credit account

    • Designation of domestic partner as primary beneficiary in will, life insurance and/or retirement benefits

    • Proof of domestic partner power of attorney

    • Other documentation that establishes formal relationship

Change in Domestic Partner Status

If there is a change in Domestic Partner status which would make the Domestic Partner no longer eligible for benefits (e.g. a change in joint residency) the employee or Domestic Partner must notify Farm Credit Foundations within 31 days and complete a Statement of Termination of Domestic Partnership. The Statement of Termination shall affirm that the Domestic Partnership status is terminated as the date of execution specified therein and that a copy has been mailed to the other party by the party authorizing the action.
